Waiver of Liability and Informed Consent

“THIS AGREEMENT AFFECTS YOUR LEGAL RIGHT. PLEASE READ IT CAREFULLY!”

This Release, Waiver, and Hold Harmless Agreement is made by and between the undersigned (client) and LiveBy Pilates Inc. including owner, teachers, interns, apprentices, contractors and employees; herein to be referred to as “The Studio”,and entered into on the day, month, and year below.

I am enrolling in a program of physical activity including but not limited to the use of various Pilates equipment. I hereby affirm that I have and will keep “The Studio” fully informed of any physical condition or disability that would prevent or limit my participation in this exercise program.  

I acknowledge that I have either had a physical examination and been given my physician’s permission to participate or that I have decided to participate in activity and use of equipment without the approval of my physician and do hereby assume all responsibility for my participation in the Pilates Method. 

I do also hereby release “The Studio” from any responsibility or liability for any injury including those caused by the negligent act or omission of any of those mentioned or others acting on their behalf or in any way arising out of or connected with my participation in any programs offered by “The Studio”. I, for myself, my heirs and assigns, hereby release “The Studio” from any claims, demands and causes of action arising from my participation in this exercise program. 

I agree to be liable for and to hold harmless and indemnify LiveBy Pilates from all actions, proceedings, claims, damages, costs, demands, including court costs on a solicitor and own client basis, and liabilities of whatsoever nature or kind arising out of or in any way connected with my participation in fitness activities.

I I hereby consent to receiving email notifications and updates from LiveBy Pilates.
